We partner with Alma to make using your insurance for psychiatric care as simple as possible. Your insurance eligibility and estimated cost will be verified through Alma before your appointment.

These plans are not yet available for insurance scheduling. Returning patients with UnitedHealthcare/Optum/UMR may contact Revive and Thrive Wellness regarding the priority waitlist or available Returning Patient Self-Pay options while credentialing is completed.

Private-Pay Rates
Initial Psychiatric Evaluation (60 min): $300
Follow-Up Appointments:
• Medication Management (20–30 min): $150
• Medication Management + Therapy (30 min): $180
• Medication Management + Therapy (45 min): $225
Superbills are available upon request for patients who wish to seek possible out-of-network reimbursement from their insurance plan. Reimbursement is not guaranteed.
Wanting to Re-Establish Care With Nicole?
If Nicole has treated you previously and you'd like to continue your care with her at Revive and Thrive Wellness, welcome back!
Revive and Thrive Wellness is a separate private practice. Even if you've worked with Nicole before, you will need to establish care with the new practice before ongoing treatment can continue.
When scheduling, please select the 60-minute Initial Psychiatric Evaluation as your first appointment. Please do not select a follow-up appointment.
You may use an accepted insurance plan, or eligible returning patients may choose our Returning Patient Self-Pay Rates:
Initial Psychiatric Evaluation (60 min): $200
Follow-Up Appointments: $130
Important Medication Refill Information:
If Nicole previously prescribed your medications through another practice, please be aware that refills cannot be provided through Revive and Thrive Wellness until you have completed your initial evaluation and established care with the practice. Please schedule your initial appointment before your current medication supply runs out.
What Happens After You Book?
Once your appointment is scheduled, please keep an eye on your email for important next steps. You may receive several separate emails and notifications as we prepare for your appointment—this is normal!
1. Revive and Thrive Wellness Intake & Consent Forms
You will receive an email containing the intake and consent forms required by Revive and Thrive Wellness. Follow the instructions in the email and use the provided passcode to access and complete your forms.
Depending on when your appointment is scheduled, forms may arrive shortly after booking or closer to your appointment date, including within 24–48 hours of your visit.
All required forms must be completed before your appointment to avoid rescheduling.
2. Patient Portal Invitation
You will receive a separate email from Revive and Thrive Wellness inviting you to activate your secure patient portal. Please complete your portal registration when you receive it.
3. Insurance Platform Communications
If you are using insurance, Alma or Headway may also send separate forms, insurance or payment information, appointment reminders, and automated notifications. These are separate from the forms sent by Revive and Thrive Wellness.
Please complete any required forms from both Revive and Thrive Wellness and your insurance platform, as they serve different purposes.
